I have a new Nano +. Same issue as you have described but I did nt have the foresight to try to power off the RC and then back on to see if it would reconnect. Mine would not RTH. It just hovered where it was. Fortunately I could see it and the controls worked even though it said disconnected...

A bit more info; even after the drone disconnects from the RC, the green light remains on, on the drone.

I have the same problem. I was about 300 yards away at 130 feet and it disconnected. Good thing I could see it because it did not try to come home. It just stayed where it was. The green was frozen at the point that it disconnected. I landed ok and tried numerous times to turning off and on...
